You will need to download and complete an intake form to help me better understand your situation and an terms of acceptance form, these forms must be filled out and returned to me before your session.

I also require a $100 non-refundable deposit before you have your session.

Take some time to fill in the the intake form - your answers to the questions will help me to tailor your session to your exact needs - so please be as thorough and accurate as you can be.
